The person in need of assistance contacts the Lions Club, and a referral is made to the
Lion in-charge of gathering income and need information from the requesting individual.
The Lion will contact the requesting person and gather needed information.
The Lion will complete the form over the phone, and mail to requester for signature.
Then the requester takes the form to VisionPlus, and they will help with glasses or eye exam.
Vision Plus keeps the form and sends the bill to the Eye glass Lion for approval of payment, then
the eye glass Lion approves for payment and mails it to the Lion treasurer for payment.
